SW London 18 - 25 Community Respite Service

Introducing our new respite service

The responsibilities and challenges as a lived experience for carers across the UK are as the Carers Trust has consistently identified one of too often being unseen and unheard. Our social care system is overly dependent on the time, dedication and and enormous emotional consequences of carers to do what they do without (too often) recourse to support from the state or its partners.


Recent research from the health foundation found that the number of young people registered with a disability would grow by 35% from its current level by 2027. This kind of growth will only continue to increase the demands on unpaid and undervalued carers. With options increasingly limited for meaningful and flexible respite options, and substantial gaps in capacity against demand from those with caring responsibilities the charity is offering a range of potential options to address local needs.

The Balance Response

  

The Young Adult Respite Service will provide respite and support to individuals who are living with a learning disability, autism and/or experiencing mental health challenges. Our target audience will initially offer services to:
 

  • Individuals aged 18-25 years. 
  • Individuals registered and supported through an Education, Health and Care Plan (EHCP)
  • Registered with or known to Achieving for Children or other specialist statutory providers of children and young people’s services in SW London.
  • In continuing education and training provision

Each package of care will provide tailored, personalised support, promoting independence toward enhancing the overall well-being of clients. Through a combination of structured activities, counselling, and social care support, the service will empower individuals to develop coping and daily life skills management, improved confidence and better quality of life. It will also provide essential breaks from caring responsibilities for families and carers, with access to advice and support from the charity's skilled practitioners.

The service will initially operate as an out of term-time offer only, unless there are clear exceptions that are deliverable within the charity’s existing capacity. Interventions will be based around the following community based options:


  

  • Outreach based respite support: Structured respite support based on our outreach model delivering daily living skills, training allied to pre-existing learning goals, support that builds on their existing strengths and knowledge as well as integration with existing activities that individuals may be involved in. 


             This will operate as both a weekday and weekend service between 0800 - 2000.


  • Day based respite support: Operating from one of our community based day opportunities service sites in Wandsworth or Richmond providing horticultural and/or other place based mixed activities and support. 


             These are currently a week day service only between 0900 and 1500

Access to and paying for the service

Access to this service may be direct through our Community Support Service Team as a self-referral or through your social worker and/or Community Learning Disabilities Team, of the Community Mental Health Team. You may be able to access funding to support a temporary (or longer term placement) with any one of our services, but will need to discuss this with your Care Manager.


Options for private payment are also available based on the following rates:


  • Community Based Outreach:   £30 per hour or £180 per 6 hour day
  • Day offer Horticulture Service: £54 per day, or £30 per hour where one to one support required
  • Place based Day Opportunities: Group based purchase £125 per day based on 6 hours
  • Place based Day Opportunities: One to one purchase £37 per hour or £197 per day based on a 6 hour day.
  • Transport: Transport is offered to our place based services, though this also incurs a charge.
