How we work and what we do
Our outreach Service is a specialist community based provision supporting people with a range of complex support needs including learning disabilities, mental health illness or autism living in SW London.
- Our outreach service is customer focused and person centred and is there to help with any aspects of your life where you may need support.
- Our diverse and highly skilled staff team provide person specific and targeted support. We make every effort to ensure that people are matched with someone who they are comfortable with and are happy to be supported by.
- Our staff team will encourage, prompt and support you in a way that you want and when you need it. We can provide support in all locations, including homes, workplaces, community centres or hospital settings.
- Our staff will provide support from a couple of hours a week to a more intensive level of intervention if required. They will help build skills and confidence in realtion to the general challenges of daily life. And will provide advice and guidance on money matters, managing your home and relationships with other key services you may be engaged with.
How to access our service and its support
- You can seek referral through you local Adult Social Care Team with the costs of support met through an individual care assesment.
- You can talk to us about a privately funded referral. Costs will vary depending on the level of support that is required but start at £28 an hour.
- You can come and talk to our service managers about what the service does and how it does it before making any decisions.
